... Where was the full OP first seen? I thought it was on the Evangelion Test Type DVD, if my memoir serves.

That is correct. It was an extra on the Evangelion Test Type DVD. I believe that's the only place it can be found (not including recording it into a video file for people to download and watch on a media player).

The full Op is awesome. About the mask on Lilith's face, based on the clearer pic of it in NPC episode 22, I assume that her mask in that pic is not the same as the purple Seele one that she has on the cross. It is black(or very dark), has no eyes, and covers a bit more of the face. Screenshot? Also, Lilith looks very squished in that pic.

Actually, the eyes are there; they're just very difficult to see. The area of coverage is the same.

No, during the actual mind rape, with the fully armored Shogouki. (Right after Asuka says "Why are YOU there! "You won't do anything!" "You won't even hold me!" I think putting that flash there for a sec seemed a little out of place.
