I read several times in Reddit and Instagram comments about this.

Supposedly this happened between the production of NGE and the beginning of the production of EoE. People claim that Anno had a crush in her and Miyamura rejected him. Other examples claim they even have a relationship that ended bad. But the worst part is some people theorizes the Hospital scene was a kind of "revenge".

Sounds creepy but I want to know the true.

It isn't. There's an old fake ex-employee leak/rant with a name I forgot that suggested it, but it's fake.

No. A lot of people try to push these rumours, sometimes as part of the weird narratives around Anno and his "revenge", sometimes for shipping reasons. You'll see this used with misquotes, rumours, and all sorts of exxagerations, mixed with partial information and speculation presented as fact (though often it's simply echoed by those unaware of the facts). Some fans are *obsessed* with it for Ikuhara/Miyazaki as Kaworu, or Kotone as Misato. Recently a lot of people have made that claim for Moyoco as Mari, which was strongly rebuffed by Khara staff and the Annos themselves.

Eva fandom actually has a long story with this. The Kaibunsho, a now widely discredited slander document, famously claimed Anno was rejected by Nadia's seiyuu, Noriko. Don't take it seriously, but it is a relevant piece of fandom history.

Miyamura herself is also the victim of additional misinformation surrounding the EoE choking scenes. People try to claim Anno forced Ogata to choke her, or even did it herself because it was his fetish. This is nonsense, but it was spread by a widely read Tumblr post (it's always them). It is addressed it in the wiki here.

Now, I wouldn't say it was impossible. In this interview, Anno and Miyamura sound outright flirty. The rumours are old and in one case Carl Horn (manga editor in English) asked about a staffer about it but he simply replied "where did you hear that?" But there is no evidence of it so let's not take it as fact.

The examples Felipe gave are some of the more prominent cases, but there's dozens like that; any time you hear a "akshually [writing decision] was because of staff drama", that should cause every alarm bell in your head to go off at the same time

Now I can kinda understand when people say that Anno hates Evangelion’s fan base, if they are just making up random bullshit about him that isn’t true behind his back.

All of its basis is an idea of "oh this end was so confusing, clearly he must hate us innocent fans who just want a cool action show", and some largely misquoted interviews from a time where he was known for giving intentionally confrontational answers to what he (presumably) percieved as stupid questions
