People get aroused by video of monkeys fucking - that's not because they want to fuck monkeys, but because they realize that it's a sexual situation. Maybe they imagine what the monkeys must be feeling.
A lot of ppl who consume all this 'high school setting' material are nostalgic for their own youth or an idealized, much hotter version there of that they feel they missed out on.
Ecchi is just a genre that exists, maybe not the most highbrow thereof, but, most reasonable people get that it's not any more realistic than fantasy or action or full on porn for that matter.
Let's also consider that we're not discussing grade schoolers but individuals who for most of human history might well have been considered adults depending on the local cultural norms - you could ascend the English throne at 15.
Everyone gets this with physical violence somehow. Of course we can discuss wether a piece of media feeds into unfortunate social trends, like, say, "why are the baddies all middle eastern" or "should someone who is portrayed as a hero do pushy, inappropriate behavior", but everyone gets that enjoying lavish, detailed images of murders in a consequence free fantasy setting doesn't mean you want to go murder real people.
