Non-written fics
I was gonna' ask what fics pairing have been done, but instead I think an easier question would be; What fic pairings haven't been done?

I can't say that I've looked exhaustively. I definitely haven't seen Kensuke paired off all that often, and certainly not with any of the adults (I think he gets paired more in pics than fics).
My favourites - Asuka/Hikari, /Maya and /(boy-)Kaworu - have been done, but are as rare as hens' teeth.
Of course this is without crossing the series barrier in crossover fics (like the wedge of unwritten ideas I have for anyone who's interested - http://www.stevegilham.com/nge/variations.html )

Easy. I haven't found ANY Evanglion relationship fics!

Unfortunately Reichu's "Beyond Eva's Shores" is caught in eternal draft state. There is, of course, her sculpture of Santa Barb and Nao-chan, which arose from the scenario. Look for the "Naughty Evas" thread

R/K is definitely a favourite for pics. As for Kensuke/Asuka - - she doesn't have a lot of choice, does she. I did pitch a scenario with this in on ANF last year in response to "What if EVA-04 was delivered OK?" - which had EVA-02 fall victim to Bardiel, and with her being invalided out, Asuka and Kensuke were thrown together while everything else happened.
