Well.... look at her skin! She does look like she would smell like a hospital, does she not? After all, anytime you saw her on a hospital bed, she kind of formed a visual symmetry within terms of colors with the hospital bed and bandages. And that is precisely how she was introduced into the series.

I was always of the opinion that Misato was "cool". Everything about her looks cool, especially when she wears denim. Denim is the iconic definition of my interpretation of "cool". Or that time she showed up for Shinji's parent teacher meeting. That entrance with the car, the outfit, and the sunglasses was just way too cool.

I thought that in the context of the series Japan's overall climate had been altered to the point where it's now basically summertime all year long (due to the Earth's axis shifting as a result of Second Impact, or something).

Generally hospitals have a pretty unpleasant smell of sterility and cleaning product - I don't know if Anno means that she smells like sick people (I hope not, good god) but when somebody says that something smells like a hospital, that sort of dry sterile smell is what I'd associate with it.
