Unknown Asuka image, used in official merchandise
This Asuka image has no known artist or source, but was featured in an official calendar sold by Gainax. That's it, that's all the information i have.

It looks like you found the source. It's official art originally made for the 1999 calendar.

I have known about this illustration for several years, as well as the other illustrations in that calendar that also belong to the same "mysterious artist". I tried at the time to find out who the artist was, but was unsuccessfully.
In some places, these images are mistakenly credited to Sadamoto. These illustrations are also not included in any artbook, not even Die Stern, which has all the illustrations from the 1998 calendar.
EDIT: this is also a problem with movic (a Japanese anime and related media company): all the Evangelion calendars printed by them do not credit the artists, IIRC.
