Is there a clear definition of what counts as fan-service anyway? It seems a very vague concept to me; and while there are many obvious examples, there are also many cases which are quite uncertain quite apart from the matter of differing cultural sensibilities (I mean within the West, not just between the West and Japan).

Well, I'm watching Railgun S-1 right now and I'd say EP 13 probably takes the cake for Anime I've actually seen.
I mean, predictability is the central attraction and the narrative hook that we've all come to expect from the Evangelion franchise. How come Anno can't realize this? Twice? - FreakyFilmFan4ever

In my collection, the most blatant is Golden Boy. Curiously, and to my embarrassment, that was my first introduction to anime, so it's hardly surprising that it was a long time before I was persuaded to take the medium seriously.

One that comes to mind is Green Green. I actually felt a bit sick after watching it.

I'd have to go with ADV's release of Plastic Little, which broke bold new ground by first introducing the on screen 'jiggle counter' special feature.

They re-used that for Burn-Up/Excess (and possibly /W as well); but I still wouldn't list those titles under this thread, because they were really quite tame.

I've recently watched season one of Highschool DxD, and I'm surprised no one's mentioned it yet. It's a show built solely around tits.
The BDs aren't even censored, so you can hardly go a minute without huge naked breasts bouncing everywhere.
And the cut-away screens look like this:
