Correct. It's Kafuka Fuura from SZS. Specifically, I got her out of the Bangaichi OVA version of Ringo Mogire Beam:
AKA one of the best, most fucked up openings in the history of anime.

@Nuke: it's an understandable mistake; the OP linked above which I took the avatar from was done by the same guys who did all the witch/familiar/labyrinth designs for Madoka.

Is the show any good? I've seen clips of it and it always looked to me like Moe generation nonsense. I may have just really seen the wrong clips, if that opening is anything to judge by.
