This isn't a Rei that appeared for Asuka; it's an incarnation that appears just long enough to Shinji to realize it's there and to look its way, at which point it vanishes. Personally I'm inclined to think it's a special property of the Spear and its doppelgängers to transfer damage onto the Eva pilot, probably connected in some way with its ability to break through AT Fields.
