I am not certain if this has been observed before but listen to this:
Remember when Kaji brought Gendo the Adam embryo? Kaji right then knew what Adam was and what Adam looked like. He literally saw Adam in a embryotic form.
A few episodes later, Kaji opened Terminal Dogma and showed Misato Lilith, which Kaji said was Adam.
So, keeping in mind that Kaji already knew what Adam looked like, how could Kaji mistake Lilith for Adam so easily?
P.S. If you look on the NGE: D & R DVD cover, Rei's left hand only has 3 fingers! Silly Sadamoto....

Its understandable. The appearance of the Angel in embryonic form is no indication of what it will look like when it gestates. Kaji most likely presumed that Adam was allowed to gestate to a mature state for various research and experimentation. He knew nothing of Lilith, so logically in his mind it had to be Adam.

It's very difficult to see, but she does in fact have the right number of fingers. One is slightly obscurred behind the other, which is why I've indicated in red.

Definitely not "where is your"... I thought that maybe this was, at least partially, German ('Heis' starts the second line; not sure if that actually means anything, but it SOUNDS German). Third line says "why?". The notes on the right below CAUTION look Japanese to me, but that's the most I can say.
